Minnesota Senior Health Options (MSHO) and Minnesota Senior Care plus (MSC+) program
St. Louis County Public Health provides care coordination services for residents of St. Louis County over the age of 65 with Medical Assistance eligibility and are enrolled with Blue Plus Managed Care, based on the primary care clinic.
Care Coordinators assess individuals to determine their needs and what services, if any, are necessary to keep them in the least restrictive environment, whether that be at home, in foster care, assisted living, or skilled nursing facility. Care coordinators can:
- Help answer questions about available services.
- Work with providers to help people get the care they need.
- Help providers share informati0on with each other.
- Ensure people get home and community-based services they need.
